This is the order form for a shirt that will match your child's 2026 Flag Football team. The front of the shirt will have the league logo, as well as match each child's team's shirt color. Refund requests will not be accepted. Once the order deadline has closed, shirt orders will not be accepted. We are opening this up now so that you can have the shirts as soon as possible.

AGREEMENT TO HOLD HARMLESS AND INDEMNIFY
The undersigned below, by executing the Agreement To Hold Harmless and Indemnify, hereby agree that we will hold harmless and indemnify the City of St. Joseph, St. Joseph REC Center, MO Family Sports, its organizers, coaches, players, agents, members, partners, and volunteers, from and against any and all claims, damages, expenses (including reasonable attorneys' fees and expenses), lawsuits and injuries to person or property arising our of or related in any way to MO Family Sports.

This Agreement To Hold Harmless and Indemnify specifically includes all claims or damages to persons and property.

The undersigned hereby acknowledges that MO Family Sports may not carry medical pay insurance coverage or any other liability insurance coverage for any accidents arising out of or related to this event.
