This form is for teams joining the 2026 Fall Basketball league. PreK to 1st-grade teams can be coed. 2nd-6th grade will have separate boys and girls divisions.

The cost of registration is PER PLAYER. You must change the "quantity" to match the number of players on your team. For example, if you have 8 players, then your quantity should read 8.

Registration does not include team shirts. Your team will be responsible for providing jerseys or uniforms for themselves. Teams can opt in to ordering team shirts through us, for an additional $10 per shirt. View the Shirt Order tab on the form to order your team's shirts.

Team requests and schedule requests are allowed. We will do our best to accommodate any and all requests. Not all requests are possible.

To make a request, please email us at mofamilysports@gmail.com or include your request in the indicated section on the registration form.

- If you are requesting to play on a team, please include the name of the team's coach, the age division, and team name.

- If your team is making a schedule request, please be specific. You will also be able to make more schedule requests up until the end of the 48-hour request time.
